Currently I'm just running the sponges and ceramic cylinders, and I do my maintenance about every 4-5 weeks, and I haven't lost any flow. My filter is over 2 years old as well. If using the fine pads or floss, you need to do the maintenance more often, especially if your fish make a big mess, or you have plants that have some plant matter rotting and gets taken into the filter. My recommendation is cleaning it about every 3 weeks, if using the fine pads, or even every 2 weeks.

The easiest way for me to tell when a can (or even HOB for that matter) needs cleaning is when it/they start dropping flow rate. Fine pads make it happen sooner, but eventually it will slow down no matter the media configuration.
Set it up the way you want it media wise a clean it accordingly.
