Wrong set up for the flowerhorn by the way...treat him in a hospital tank with melafix + pimafix + aquarium salt at 88-90 degrees. Have him in his own tank next time, build up his confidence, and he'll look way better with less damage to himself.

With all the stuff in that tank it must be next to impossible to keep clean. Pull everything and do a HUGE water change.

everyone will say it lol that that is from bad environment. do what big one says and give him his own tank so he can feel better
