Some one please help the life of a great fish is in danger

My FH HAS WHAT I THINKK IS POPEYE I added a pic

I treated him in a hospital tank with ocean free super internal bacteria for 4 days with no improvement
I have been giving his 30 min epsom salt baths for 2 days 3 tbls per 5 litters.
But he is still not getting better he wownt swim around just on the bottom not moving just fins are moving a little
Help what can I do ??

Oh and my tank was clean I just didn't vacuum the crap up for about 2 weeks

Thank you my Freind's aquerano but he passed away yesterday it turns out he had dropsy and not popeye. Thank you for your input...

Right now im really sad cuz I loved him and now he is gone. I thought about replacing him but I need some time to get over this sad loss.

I am gutted to be honest. My tank is pretty clean and I have three internal filter and one huge external filter for my 500 litter tank and I clean it once or twice a month... But as for the external filter I haven't clean it for a long time. Do you think that is why he got sick. The external filter is a tetra 600 litter filter
