How does a fish even get to that point? How could you see an animal in your care look like that and not care to do anything about it?

Looked like extreme hole in the head disease. But according the guy who rescued the fish from his Dentist office tank. It had been burned by hot water by accident. Seems the fish was hiding inside a piece of driftwood they took out to clean. Poor fish..
