Thank you all for all of the information! I will definitely get a test kit and test ASAP and try to get a tank for quarantine as soon as I can. Will post any further developments. Thanks again!

It's not water parameters, I can say that much. You should get a test kit anyways, they're good to have.
It's definitely some sort of internal parasite. Keep doing what you're doing, and get medicine to treat the water for parasites.
