Emergency: help needed with swelling on disk

Problem you are having:
1. A few days ago I noticed a small swelling/welt on the underside of my rays disk. No apparent redness but over the past 3 days the swelling has increased. It has not seemed to have effected his appetite or activity levels but I'm worried this may be something more serious. First day I noticed I did an 80% water change. Each day since I've done a 25% change
I first noticed the issue the day after I added the manzanita wood.

Water conditions

Ammonia- today: 0.25ppm/0ppm
Nitrite-0ppm/0ppm
Nitrate- 25ppm/20ppm
4. Water temp? 79°F
5. What is your water change schedule (how often and how much) ~75% on Wednesdays, 25% on Fridays & Sundays


Tank setup info:
6. Type and depth of substrate?
Pool Filter Sand ~1"
7. How long has your tank been set up?
~4months (used seeded media, prime & stability to kickstart)
8. Method of aeration of water
1 1600gph powerhead/ two circular air stones
9. When was the last time you cleaned tank and substrate?
Today. Typically cleaned on Mondays and Wednesdays
10. What size tank do you have?
150g (gallons/liters and footprint)

11. What type of filtration are you using in your tank?
SunSum Canister (525gph) & seeded lava rock/filter floss in overflow
Please post picture of your tank ray is housed in. (if able, this helps us see your tank set up and rays living conditions)

I've kept saltwater rays for years but this is my first freshwater ray

Less than a year. Exact age is unknown age. Male.

Heater burn? Kind of looks like ammonia burn to be honest, keep up the water changes I would test prior to water change to see what ammonia levels are at I'm assuming you tested with old kit it read .25ppm then again with a new kit @0ppm?

That is correct about my ammonia tests. I had wondered about ammonia burn but it's so localized that I wasn't sure. It's not like ammonia burn as I've seen it before. I'll keep testing for it and keep up on the water changes. Been dosing with prime daily as well just to be safe. Also, not sure if I've mentioned it but my heater is covered.

My heater is covered. But I suppose there is a chance that he still could have burned himself somehow. Heaters have been moved regardless.

Do you believe this is heater burn? As it's seeming to heal it also appears to be getting larger. The swelling itself has gone down but it seems to have increased slightly in diameter while taking on a very slight yellowish appearance.

Thanks for the tip. I'll see if I can get a hold of him.

This whole thing has really confused me. I'm having a hard time figuring it all out. It doesn't quite seem like anything I've seen before. It almost looks as if it is some type of allergic reaction to something.. somehow haha
