75g tank trouble - constantly dying fish

Hey guys, i'm posting this on behalf of my mom who can't seem to figure out whats going on with her tank. She has a 75g tank, it's freshwater, she's running an emperor 400 for a filter, and her tempature is about 78.

Her current stock list is:
6x gold tetra
7x rummy nose tetra
6x neon tetra/cardinal
2x glowlight tetra
5x bloodfin tetra
4x swordtails
4x platies
1x small rubber lip pleco
4x oto cats
2x peppered cory cats
3x albino cory cats
3x julli cory cats

Her water is a pretty a steady 0 ammonia, 0 nitrite, 5 nitrate. A few days ago a panda cory's eyes started to pop and then died pretty quickly + she lost an oto cat, and then the other panda started not looking so well. Then she lost another oto and a sword a few days ago. Yesterday morning she lost a glowlight + a cardinal, a panda died over night, and sometime in the last few hours she lost an albino and a peppered cory. With the exception of the pandas all of those fish were acting happy & healthy less then a hour before they died. She also lost about 5-10 other fish the previous week. She can't figure out what's going on and it's pretty upsetting to her to wake up/come home to dead fish almost every day.

Any ideas as to what the hell is going on in her tank and how to stop it?
 
How long has the tank been set up and running with fish in it? Is she purchasing all the fish at the same location? Any new decorations/gravel been added recently? Maybe, double check the temp with a different thermometer, and check to see if a dechlorinator is being used. Start with the basics first.
 
Also what type of dechlor? I have chloramine in my water, so I have to use prime (or something that will take care of the chloramine properly).
