I’m not sure what to do any ideas. Hasn’t eaten in a month parameters are good and don’t know if it’s a swim bladder issue or not.

Question is anything missing from the tank by chance? It seems like it might've swallowed something and it got impacted bowels, atleast from how it's stomach looks. If it hasn't eaten for a month and it still has a noticably decended stomach it probably ate something that it shouldn't have.
Also if you can take a water test and post the results it would greatly appreciated, along with any info leading upto when this all started.

I’d add some Epsom salt 1 tbsp per 5g. Add a round of kanamycin. Reduce the water level so the fish can stay toward the bottom.
