Any ideas what's going on?

This guys has been looking stressed the last couple days. He appears to be dusty and fins have looked a little clamped. I've never seen this before...
Hopefully someone else has some experience with this. Here is a picture

Sorry I didn't post water params. Forgot to. Everything is good.
Ammonia is zero
Nitrites-zero
PH-8.1ish
Nitrates-about 20ppm maybe a little less

What is velvet? Never heard of it. His mouth also is starting to look like it's locked open.

Lower the temp in the tank below 78, add one tablespoon of salt per 5 gallons of water and treat with Furan 2. If you cant find Furan 2 use API Triple sulfa. Furan 2 can also be used in combination with Kanamycin.
