Are arrowhead Pufferfish poisonous?

Wildcaught arrows are poisonous and contain tetradotoxin derived from the foods they eat in the wild. After some time, they lose their toxicity.

ANd, arrows are pounce predators. If a tankmate swims near their "stakeout" position, the puff will most likely eviscerate its tankmate.

Don't bet on it. I've had a 6" arrow gut a large 16" redtail gouramie that I only intended to keep in its tank for a short bit of maintenance. These puffers can get all the meat from a large clarkii crayfish in about 5 minutes. They bust up the carapaces like it was tissue paper.
