Hi All
My flowerhorn had a history of white poop before but I remember seeing white poop as thin long and stringy. I am not sure if these are white poop (look like shed skin material) because recently my flowerhorn has been leaving a lot of such material around in the tank. he has also not been eating much, poop very little. he will just stare at the pellet and something going up quickly for 1 pellet and dive back again.

That fish looks extremely sick. The skin also seems to be in poor condition

Increase ur water changes to 50% twice a week with a dechlorinator. Add some bacteria in a bottle to the tank. Add salt 1 tsp per g. Check ur ph. Get ur tank cycled. Then try refeeding with hex shield soaked with garlic or bloodworm juice. Give some info on ur filtration.

I have tested the water condition such as PH, Ammonia (0.25) and the rest and they are all at 0 level. Currently I am change 30% water every day and adding Paraguard from Seacheam. I cured my flowerhorn from Hexamita by giving him direct oral Epsom salt solution in the past, so I am continuing to do so just in case it is a bacteria infection coming up. I have try to searching the internet for information and nothing much came up with such symptom expect for "skin peeling" Fish has been with me for 3 years.
I started realizing this "skin" issue but was told that it is just "fading" and it is a natural thing for some flowerhorn to fade as it is a process for them to change their color. But when he start having feeding issue, I realise something must be wrong
 
Skin issues are generally due to something in the water. The fact that nitrates = 0ppm means that the tank isn't cycled anymore. Did you use water conditioner to get rid of the chlorine/chloramines from your water source?

I am using seacheam prime whenever I change water during the weekend. i didnt age my changed water if that is what you are asking. The fish has been with me for 3 years so I am pretty sure that the tank has been cycle?

It may have been at one point, I'm pretty sure you lost the cycle somehow. Nitrates should be orange on the API liquid nitrate test (not yellow). We'll need to know more information on how you maintain the tank and filters, and how often the fish was fed, what type of food, and if you did any cleanup of uneaten food.

Do you put enough Prime for the entire tank volume when doing water changes?
 
I'm not sure but that looks like a new tank. The fish might have been upgraded to a new tank and thus the tank didn't cycle. That could be a possibility cause the glass on the tank looks very brand new even the silicone and there seems to be no filter media in the pics attached.
 
Now since you mentioned I might have mistaken orange for yellow. it actually a very light color so i might be wrong. I have the filter with me for about 3 to 4 years. do you think it time to change the filter? Yes of course i put enough prime for the water changes. Nothing has change from my habit of water changes and feeding habit and out of respect I dont really think those were the possible cause of issue, because I dont think i need to wait for 4 year to see thi s problem.
if the tank is not cycle, i should be seeing a high level of ammonia as well? cause there are no good bacteria around anymore?
