Black Ghost Knife Health Issue...

In tank, my black ghost knife generally hides all the time. and for some reason i can tell its not doing well. because its not hiding... and its not swimming straight. it seems more as it its swimming on its side. I am sure it is well fed. because it ate this morning when i was feeding the others. But not only that... its swimming near mid surface area.... and its not really moving just swimming in 1 spot near the overflow filter. This happened after i did a water change today. But I do water changes almost every other day and it seems to be doing fine. At night when im sneaking up on the tank (cant see me) i can tell its fine because it swims like everywhere. Another thing is i just put some aquarium salt into the tank as well.... is it because of the aquarium salt? or maybe the water change? (this water change was a little bigger than normal.) i put the aquarium salt because my amazon sword plants seem to have this strange fungus(white cottonlike film). I read somewhere it will help...
please help!
 
My BGK reacts that way if the water changes in a unfavorable way. I once add meds without thinking how sensitive he is to it. He also swam in front of the water intake, but later would swim around. I moved him to one of my sick tanks to get him out of the contaminated water. I'm not sure exactly how sensitive they are to aquarium salt. It seems like I've read some where that they are if to much is used. I've always aired on the side of caution with my BGK. Maybe you should try doing another large water change without salt.
 
ok definitely not good. its day 2 and hes still not hiding but this time its worse. Now his fins are frayed. really bad.... I just did another water change. im hoping for the best here...
