Hi i just took pics of it. A small portion of her mouth is blood stained. It was bleeding recently. Fish is a bristlenose pleco, female, and about 3 inches long TL. Its just a regular shortfin calico.

Nitrates are currently 20ppm. I performed a WC last week. A dozen or so 50% WC. Tank is planted and has adult discus. Tank is a 100g 4foot long custom.

Tankmates:
1 synodontis hybrid
3 cuckoo catfish
1 petricola cat
2 bn pleco (1 male, 1 female)
5 discus
3 firemouth cichlids
1 hybrid blood parrot cichlid
2 female betta
3 rhinogobius candidianus
5 silver hatchetfish
2 dwarf frog

Is that lace rock in the background? Pleco may have cut it's mouth on it. It should heal on it's own. You could add some melafix to the water.
