What is your temp at? I'm battling ich right now as well, I've almost got it beat at 89 degrees, no medication

With my experiance turn up the heat into the low to mid 90's and the ich will be gone in a couple of days. Right after the ich is no longer present i would treat the tank with melafix because infections are likely.

There are some tough strains of ich out there that are heat/salt resistant. Most people don't realize the amount of meds a wholesaler uses when the fish come in... It's logical that if there are resistant strains of human disease (like how penicillin isn't very effective anymore), the same would hold true for fish disease after decades of treatment.

I was fortunate to get rid of a moderate case of ich using heat only. I kept the tank at 87 degrees for 10 days. I also added an air stone to help keep the oxygen level up. All signs of ich were gone. I lost no fish. No signs of ich since last October.

Probably more than 9 times out of 10 this works very well. Just occasionally the "super ich" pops up.
