Can anyone ID this for me?

Can anyone ID this for me? I've tried Melafix, sea salt, and increased temperature - but no permanent solution so far. Only temporary remedy so far is water change, but after a few weeks the growth will gradually reappear. It tends to grow along the dorsal end of the fish, around the face, on the pectorals, and on the eyes. Appearance: borders not distinct like ich, milky white color, growth in patches/colonies, seems to itch, patches/growths are variable in size. Any help with ID and remedy will be appreciated.

certainly not an expert in dieses but could be lymphiocystis give it a google and see what u think

Looks like scrapes and some slimecoat. Would dose with some paragaurd. Helps with external symptoms. Water quality ok?
