Looks like some skin infection of some sort.

I’d try some methylene blue on a qtip and triple sulfa.

If you have a quarantine tank move your puffer there and start medicine. Topical meth blue helps but get a good anti fungal treatment going too. Fritz maracyn might do the trick.
