In a typical tank it is only used for removing tannins or medication. So when your water isn't clear anymore from tannins, or after a few days for removing meds. Seriously, if you have adequate cycled filtration there is no need to use carbon.

+1, carbon does nothing for your tank unless you are pulling meds out of the water after medicating.
 
To answer the question about replacing carbon then it does need to replace every 6 weeks, use it or not using it it's all about your decision.
