Cichla and worms

When I feed my cichla worms they engulf them and stuff comes out their gills what's happening?

Nothing unusual,the worms squirm and move about while they are inside the fish's mouth and they eventually find the gill openings if they are not swallowed quickly enough.

It's just like a cloud sometime said it's dirt from inside the worm

Yep,the cloud is most likely the soil or dirt that the worm has in its body.If you want to avoid that you can keep the worms in a container lined with damp paper towel to keep them from drying out.The worms will spit out the dirt over a few hours.
