I would guess it's sub-par water quality, when was the last time you tested/changed your water?
 
Lol, I figured that'd be the first response, I clean and water change my tank every week and in worse case every other week, 0 ammonia, 0 nitrite, 5.0 nitrate, test water weekly also, just can't narrow down exact problem, so you say I should just make sure in clean water and all should be good, I'd like to hope that works, that's my plan because he has been treated in quarantine up to this point with salt, eryurythmcin, tetracycline, and triple sulfa, but not better.
