Cloudy water

the set up:

100 g clyinder tank
200w heater
11w uv sterilizer (jbj submariner)
Wet/dry filter
mag ? pump.

Stock:
5 rummy nose
6 rasbora
4 hemiodus
4 black skirts
8 asst corys

The problem that I have is clody water. I currently am running carbon in a media bag, have phosguard in the sump as well. the water parameters are 0-.25 ammonia, 0 nitrites and 30ppm nitrates. phosphates are at 1ppm and ph is around 7.4. Recently I have added fish about 2 weeks now and the tank has been running for 3 yrs and thats about it.I do a water change and 2 days later its back cloudy again. I have a polyfiber pad in the filter on the drip tray. I am doing the regular feeding wit no left over food and a mix of frozen and sinking tetra pellets for the cory. I am stumped for what could be causing the cloudy water. its a white cloudy rather than green. any suggestions???
 
how long has your tank been set up?
 
If it's white or greyish cloudy. I would guess bacteria bloom. Did you clean your filter too? Also if your using dechlorinator. It could be your doing too large of a water change. How much are you changing?
 
No actually this tank has been running for going on 4 years now. when I changed the water, I used prime for my dechlor and was only 25% when i did w/c every other day recently, I hardly touched the gravel, more of a replacing water only. I changed out the polyfiber filter pad and that is it.
