Clown Loach sick?

So one of my dozen clown loaches has gotten huge. But looks bloated/skinny face.

Obviously something is wrong with him/her, because none of the other loaches look this awkward .

I've dealt with the skinny disease with them, and cured it with deworming flakes months ago. But this is weird.

Any ideas? I don't want to lose him, he is probably 8 inches or so.

Do you have a hospital tank? Is the fish eating?
