Clown Loaches - Hole in the Head

I have a super overfiltered 150 ca/sa cichlid tank. I do weekly 50% water changes. I feed NLS & HBH Super Soft spirulina. All my fish are robust and healthy. Out of the blue my clown loaches have hole in the head! WTF???
Any ideas?

How many loaches lok like that? It looks like an injury from somthing.
