Dead pbass, any ideas?

Hey everyone. Today I woke up to find my pbass at the bottom of the tank belly up, mouth wide open not breathing at all (asume he died shortly before I woke up for work). Water params are perfect, but with soft water. The only thing I could see wrong was a chunk of his gills missing, is this what caused my fish to randomly die overnight? Hes always been healthy with bright colours, until i found him dead. Any thoughts?

He was solo in a 40gal cube tank. Did about 35% waterchanges every 3-4 days. Ate talapia and shrimp.
 
Fish that die with their gills widely flared are thought to have died of Nitrite induced "met-hemoglobin-emia". The nitrite binds fish red blood cells causing gasping. Gills look fine even with a chuck missing.
 

I doubt it was lock-jaw as he was alive and acting normal before I went to sleep. Woke up for work, looked in the tank and found him exactly as he was in the pics I posted. As for the nitrite thing kendragon posted, i have a pretty good routine when it comes to maintenance and water changes. I tested the water after work, and all showed up normal but with soft water (normal for my house).
