i am curious if plants both in the aquarium and in the sump like pothos remove fish medicine? do they have an active carbon like effect in our aquarium?

Don’t think so.

It may have some effect but would probably be negligible, carbon filters are designed for reducing chemicals in the water.

It may depend on the plant, and the type medication.
The floating plant Salvinia is known to remove certain heavy metals (in solution) out of polluted water, so it might reduce a medication like Methylene blue, or malachite green
e.g. Salvinia might reduce the efficacy of a treatment like Nox IcK that contains mlachite Green

A medication in the antibiotic family may not be affected, salt would also not be affected.
It might help you to google the specific type plants you have, and the components of the med you use
