When you test for Nitrate do you shake the bottles well especially the #2 bottle. I'm asking this because it's very hard to believe that just doing 20-30% wc's a week and your Nitrate reading is 5 ppm. In the top pic it looks like a scratch so I'm not sure if it's HITH.

I agree with tlindsey tlindsey shake #2 till your arm falls off.
I'd also say yes hole in the head at least the bottom pic. What's pH? And retest nitrate 5 is very low, to low.

I haven’t been shaking the bottle that hard so I my readings are probably off and ph is 6.8

You my friend have the PH that best for keeping SA Cichlids such as a Oscar. I suggest just up your % of wc's.

I looked at 2 again really close, seems to be alot of water spots on the glass made me think it was the same fish at a later date. So could be a scrape.
