Does my red devil have hole in the head??

It looks to me like it has been beat up by another fish (anterior of the dorsal). There seem to be a few scales missing but as of yet, no overt bacterial scarring (at least as far as I can see) but beginnings near the eye, and nostril. The stress of "what ever is going on in the tank" could be an omen of what is to come, unless that stress is removed.
I edited the shot a little to get a better look.
