Is it real or photochop-shopped? For some reason I thought I remembered seeing this picture without the 2nd head...

Looks photo-shopped to me. If such a fish did exist the two heads would most likely be side-by-side not stacked on top of each other.
 
100% photoshopped sir, I used to take allot of photoshop classes in college.
 
Yeah looks photoshopped to me. My dad does a lot of that photoshoping at home and it reminds me of something he would make.
 
It does look suspicious, but it may very well be real.
If it is, it's just a rare birth-defect. Dual-headed specimens have been observed in many species, so I don't see why arowana would be any different.

EDIT: After closer inspection, I can say with almost complete certainty that it's a fake. If you look at the sides of the lower head, close to where it meets the body, the edges aren't sharp enough. It looks like someone's used the eraser tool in photoshop to reshape the lower head.
 
It couldn't live long enough to grow to adult size (doesn't look juvenile). The lower head is restricting the gills and jaw in the upper head. The spinal cord for the lower head is going through the area of the upper heads throat.

I'd have to see a video before I believed it. And I agree with thorpbrian in that the heads would be side to side. In parasitic twinning, the "twin" is on the scale of weird to bizarre. In conjoined twinning, (such as Abby and Britney Hensel), you get a more biologically functional outcome.
