Dwarf Gourami SIck? Injured?

Hey all,

Noticed a little something on my Dwarf Gourami's face the other day and wasn't sure what it was. Thought it might be a fungus or possibly just an injury, but doesn't look like fungus anymore. It is looking worse and I was hoping someone could identify what it was and suggest action to help him heal. Also on his left side about half way back he seems to have another spot that occasionally has something white sticking out. Like a big pimple. Possibly a worm?

He seems to be swimming the same as when I got him, although a couple times I have seen him completely sideways. It looked like he "layed down" and scratched his dorsal fin on the grass a couple times. Also, the Pleco has been defending his algea wafer from said Gourami.

Ammonia - 0 ppm
Nitrite - 0 ppm
Nitrate - 0 ppm
pH - 7.2
Temp - 78

10% water change this afternoon

5 Zebra danios
1 Ghost Shrimp
1 Common Pleco
1 Dwarf Gourami

Penguin 100 Filter
Water heater
1 Airstone

I would appreciate any and all help. I've posted a video and some blurry photos so you can see. Thank you in advance.
