Emaciated discus with white feces vs worm... tapeworm?

I was wondering what you thought this was and what to do about it - I apologize for the picture quality but this the best my father could do. It’s from his 180 discus tank.

He has 1-2 discus that look emaciated and this was trailing one of them. Could this be a tapeworm or another type of worm / parasite causing this? And if so any suggestions for treatment?

Doesn’t look worms but more like stringy white poop from hexemita.

Metro and prazipro dosed to the tank with Epsom salt 1 tbsp per 5g. Also feed medicated food mixed with Epsom solution to the fish. Can use hex shield or metroplex.

Can use seachem focus to bind the medicated food with Epsom solution and regular fish food.

Epsom sol - 1/2cup distilled water and 3/4 tsp of epsom Solution.
