Doesn’t look too bad. I’d check ur water parameters. If all good then add some epsom salt and maracyn2. If water quality is poor then correct the water and try meds after.
 
Water is usually pristine. My typical WC schedule Is 30% twice a week (300 gallon tank). But there’s a back story.

Last week we noticed a bad odor in the room. Come to find out a Sagittae died inside a pile of rocks and it fouled the water very badly. Once I got the water quality back under control I noticed this eye issue. Do you think they could be related? I’ve been doing a daily 50% WC every since hoping the fresh water would help his eye.

So you think I should dose the 300 with epsom salt? How much should I use? My hospital tank is currently doubling as a quarantine tank so I don’t have the room to move him to a smaller tank.

Usually dose 1-3 tbsp per g. Maybe buy a plastic tote from Walmart as another qt tank.

Its going to be hard to treat in 300 gallons, because it may be a bacterial infection, and a broad spectrum antibiotic may be needed beyond just the salt.
So a QT tank is the most practical way to go,
With the average cost of antibiotics for 300 gal you could buy 3 QT tanks and all the gear.

If just using salt to treat
I usually bring salinity up to at least 3 ppt (Parts per Thousand), for a 300 gal tank, that would be at least 9 lbs of salt.
