I just did a water change so I havent tested yet.
But nitrate's usually around 10ppm.

It's looking slightly better but I'd still like an opinion on what you think it is.
 
Probably just an injury.
Keep the water quality up and it should heal.
Watch it and see that it’s not getting bigger.
