Fin rot medication?

Picked up a red Oscar at lfs a couple weeks ago. Picked the one with the best shape and color although it had a slightly ragged pectoral fin which o attributed to scrapping. However the tail fins became ragged later and pec never healed. Treated with melafix cleared up tail then came back cleared and came back etc. The pec has never shown improvement. Done messing with this want this guy healthy what's the best med for this?
 
Concerns me that the fish was afflicted in the first place who knows what may be going on internally is there any general cure all so we can start with a clean slate
 
Oscars are so often mistreated makes me take a little extra pride in providing the best care for mine
 
Anytime I deal with fin issues I use salt and general cure right off the bat, paraguard next, then get into the heavier stuff and reach out for guidance here when I cant figure out what I'm dealing with.
 
Agree with Yankeejack^^^^, good water will solve it....
 
Water is good well established tank and filtration weekly water changes. Fish was affected when I got it prob due to lfs conditions. Will the fin rot clear up in the better conditions I'm providing or will it require treatment. Thinking the infection was well established when I got the Fish. I am aware that poor water is the cause just trying to cure it at this point
