I have been fighting finrot for the past year in my 180 gallon. Ive tried medicating the tank and the disease will go away for a bit and then come back. I've tried bleaching the tank but that did not work either. Ive tried separating the fish into a bare tank quarantine and treating with the same medication i treated the main tank with and that seems to work fine.

Should i get a UV sterilizer for the main tank, or should i take out all of the rocks, substrate, and media from the filters and start over? Will a UV sterilizer do anything for bacterial finrot? My guess is that this disease is going dormant or something in the sand and rocks when i treat the tank and comes back when the fish dig around in the substrate or something but im not sure. what should i do?
 
A UV sterilizer won't hurt at all, but might not help. You can also remove the rocks and gravel, soak them in hydrogen peroxide for a day, them rinse and replace, but even that might not help without restarting the bioload in your filters. What meds have you tried and did you follow through with full treatment? What is your pH? A pH that is to high for your fish often leads to finrot. A product that I have had sucess with is Binox, made by Jungle. You can find it online through www.bigalsonline.com
