Fish bloated on one side

My pike cichlid has a bulge on one side that appears to be causing him to float on one side, he's at the top of the tank and I have no idea whats going on. Could he have a broken back?

He's not eating, he is in a newly set up hospital tank, he has hole in the head.

Update: it doesn't appear to be swim bladder related, I'm currently dosing with triple sulfa and TC tetracycline. I was examining the bloated area and when I touched it I could feel what felt like the fish spasming in the bloated area and he shook a bit, it almost feels like there is gas in there. I could use some help in figuring this out, the fish has been bloated side up for almost a month and hasn't eaten in around two.

P.S the HITH appears to be cured and the skin is regrowing
 
I'd give the tank a large water change and add Epsom salt at a dose of 1 tablespoon per 5 gallons of aquarium water. I'm not sure what the problem is but the Epsom salt "should" help if it's some sort of blockage in the digestive tract or an issue with the swim bladder. Do you have any pictures?
