I can’t imagine this guys spatial awareness is that great given the size of his hump. Looks otherwise healthy. Maybe a splinter off the wood or some other traumatic injury that damaged the skin under his scales.

In the absence of any info regarding your water parameters, water change routine and feeding, it is difficult to offer much advice except to add salt at 1tbsp/5gal and keep that concentration with your weekly water changes for 2-3 weeks 🤷🏻‍♂️
 
Add some salt to the tank. I’d do 1 tsp per gallon. I’d try applying some methylene blue or hydrogen peroxide to the area via a qtip daily.
