Flowerhorn head deflated

Please help me my flowerhorn head is deflating with wrinkles. What to do

is he recovering from being sick?, that sometimes happens, could be a minor injury, i have heard of this happening and fish always recovering, i would start with checking water parameters

I have changed water and did deworming also thinking he would be constipated. Hopefully he would recover soonn. Actually it happened all of a sudden overnight. Can it be a overfeeding problem? He wasn't sick at all. It was overnight
