flowerhorn scratching and brown spots..velvet?

my fader female flowerhorn has been scratching a few times in a day since last week..today the scratching has increased and i can see weird newly developed brown spots on one side of her which she has been scratching...plz see the image attached..is it velvet? how should i treat it?
i have two of the following meds with me---
1) Fish Medicure Blue liquid (Treatment of fungus, white spot, tail and fin rot)
2) Sera Costapur treatment for Ich and other parasites ( Contains : malachite green oxalate 0.18g ; Formaldehyde solution 5.89g ; aqua purificata ad 100 ml )
Do i need to remove sponge filter while treatment? Medicine Bottle instructs to remove carbon filter..

may be the brown spots are just his old pattern which is still fading .. but m not sure... i read in one thread that velvet symptoms are brown spots

Velvet will typically cover the fish completely. And is usually yellow. You could try a water change, turn the temp up to 86 and treat with aquarium salt before using chemicals. When using meds only remove carbon filtration. Keep everything else running. I'm no expert but this usually works for me. Good luck

Ok thanks i wills try that...she is scratching at least once in 2 hours... m worried.. :( how much salt should i put per liter basis? I put a handful of salt for every 30 % weekly water change..and initially i had put 3 handfulls if i remember correctly...it is a 15gallon tank
 
Not sure what that is but if its flashing and hitting rocks its not a good thing... Not sure how much pet liter but a spoon for every 5 gallons is appropriate.
 
A heaping tablespoon per 5 gallons. Give that a few days and see if the fish looks better. If not I will add another dose the same. Should see a difference then.

my flowerhorn is still behaving the same way but very healthy and active..its a 4-5 inch fader female..i tried fungus treatment with methylene blue then next week salt treatment and then finally external parasite treatment with formalin+malachite green medicine...but it is still flashing occasionally...
my tank is small, 15 gallons, but i do weekly 30% water change...i dont have water testing kit as it is very costly here..i have a small sponge filter..i m thinking of buying an internal power filter...may be ammonia is irritating my fish ?
its been three weeks since it is flashing.. :( its breathing very well so i dnt think its flukes
