Flowerhorn skin issues, please help

My 12" flowerhorn has been experiencing slime coat problems for the last few weeks. I have tried heat and salt which seem to temporarily alleviate the issues, but it always returns. My water parameters are always good, but the condition seems to get better immediately following every water change, only to return in 3 or 4 days. Please help!

i may not be right but it looks like ich to me, my female jag got it once and i asked a local pet store and they gave me this ich medicine and i put her in a medical tank and she was fine after a week. Watch out and from my experience read the entire bottle because i missed a sentence and it said it kills bio filter, plants, raised nitrates, and killed crustaions it also turned all my rocks blue. ruined everything.
