If there is a bacterial infection like columnaris, an antibiotic that targets gram negative bacteria is needed, something that treats gram + will be ineffective.
Ick is a protozoa, and antibiotics don't work, you would need something that breaks down the cell wall of protozoa osmotically, something on the order of malachete green.
If the problem is anchor worm (Lerneae) you need a Chiten inhibitor. Because anchor worms are not worms, but are crustaceans, the chiten inhibitor prevents them from developing a shell
Temperature can also determine out come.
Where for ick you need to raise temp, certain bacteria like columnaris become more virulent when the temp is raised above 82'F, and can hasten the fishes death.
