Gourami Hole in head?

Yesterday I lost my one of my gouramis, he/she was looking good earlier and the weeks before. About maybe 3-2 weeks ago I saw a white spot on its head, I thought it was a scratch or some other injury, so I wasn’t really worried and thought it will just heal. Yesterday I saw it dead and the “white spot appeared as a hole in the head.
I tested my water there are a lot of floating plants and other plants to take in nitrates, but it’s 0 or 5>. Tank is a 5.5 gallon, was 3 sparkling gouramis now 2, 4 boras rasboras don’t know the exact species, 1 oto, 2 amano shrimps, and 1 nerite snail.
I don’t have a picture of the gourami before it died but I have one after.
I do water changes from 20-50%.
Probably should have taken a picture before I freeze it, there is a white/red mark which was the hole.

That tank is way to small for that stocking you need a 20 gallon or more in order to house all of those fish together and it's probably a big water quality problem.

I wouldn’t say it’s too many fish, most of the fish only the shrimp and oto are an inch. The gouramis will get less than an inch and the rasboras genus Boras, are dwarf rasboras usually only reaching up to an inch. The nerite and oto are for algae cleanup. Tank is densely planted.

I don’t know, I tested it, I have a lot of bio media, I checked the instructions.

To me this sounds like ich. When I mostly had guppies they were very prone to an ich outbreak, whenever the fish would be finally cured of ich it always left a red and white divot on the affected area, kind of like the fish had a small bite taken out of it. But yeah it basically developed the same way you described, at first it's just a white fuzzy looking spot.
