do you guys worry about vacuuming under the live rock and the hard to get places behind them?
 
You can use a turkey baster or small power head to flush those hard to get areas out, and then nab whatever was underneath there.

Not really. Nothing can get under the rock if it is set solidly in the sand. Leaving a small percentage of the sand un-vacuumed isn't going to case many problems as long as you do a good job in maintaining the open areas and a majority of the sand.

Keeping your tank high flow with little to no dead spots will also help with this issue. It will keep wastes suspended and moving around to be filtered out.

sounds good. I have very good flow and whatever food flows to the back just about always runs back to the front and gets eaten. it was more the sand just behind the rock I was worried about.

Then you should be just fine.

+1 to Heath as well. Good idea if you see an area that is collecting stuff.
 
I dont...unfortunately none of us are perfect. I dont want to move and disturb anything that might not be doing any damage...so the rocks stay put.

I do pick up big corals however, to clean underneath them.
