Help! C. Frenata with HITH

Help! My old male pike (around 8 inches, had him for three years) has been battling Hole in the Head for the past six months or so. After doing lots of water changes, altering his diet from just krill to a combo of krill, pellets, and earthworms and using a vitamin supplement, it subsided a bit, and he started behaving more normally, but in the past week or so it's really come back hard. Lots of pitting on the head and along the lateral line, and some fin rot. He won't eat anything and is just sitting immobile, most unlike him. I tried feeding him some krill and pellets, but no dice. Going to try to get some earthworms tomorrow, which I'm hoping he'll eat. Anyway, does anyone have any advice? For some reason, I can't post pics of him...any idea why? Thanks in advance, Willy.
 
There is quite a bit of debate about HITH. Some say poor water quality, some say poor nutrition and some say carbon. I have also heard that low ph can make them more susceptible. I don't know for sure of the cause. I have heard of people who had success with Metronidazole, but I hesitate to say for sure as I have never had to deal with HITH. Hopefully someone here can help or try the disease forum, or try a search for HITH.
