HELP, Need Diagnosis.. pics included

Its about two years old and lives with 2 other oscars that are the
same size (9-10") in a 125 with 2 emperor 400 & a fluval 304.
My ph is 6.6, nitrates are 10, ammonia is 0, nitrites is 0.

I've treated the tank with metronidazole, minocycline & erythromycin
all at different times. I also fed them vitamin soaked pellets and
varied their diet with krill, worms, and algae discs. My 2 other Os
has HITH also but this guy is the worst and there are these spots
all over his body and tail. Plus one of its fins is rotting away.
I've also tried 2 weeks of Melafix with little sucess.

I recently started changing the water more often - like 20% twice a week.
I was doing 25-30% once a week.

All suggestions are welcome.

It certainly looks like hith ar a similar protozoal erosion, It looks like you are doing most of what you can. I would make sure you thoroughly gravel vac the tank or even go bare tank while they heal as the gravel can harbor pathogens, also go ahead and do 25-30% twice weekly on your water changes for a while.
Now for direct treatment here is an very old school method of starting treatment, using a plastic bag catch the fish and quickly swab each lesion with iodine solution (the same stuff people use for minor cuts), folow this up with a treatment of the tank with an antibiotic again but this time use a sulfur based one like tmp sulfa. this is just in case you have a dual infection going on. Remember to bypass the filter during this phase, use sponge filters and airpumps or power heads. Then finally repeat the the metronidazole treatment.
I was recently reading of hith lesions often culturing positive for both hexamita and gram-negative bacilla so if you treat for one the other keeps the wound open until it is recolonised and vice versa.

I have been struggling to control, HITH on my pike-I see now one of my problems may be diet related as it is not that varied.

1.try slightly raising the temp to 80 and use some aquarium salt

2. and i would get a uv sterilizer if you do not already have one. I know this makes you angry to see your fish like this but you have to kill the Bacteria and parasites. 3. You should also check water parameters to make sure the ph isn't going up and down. If you are adjusting the ph more than just 2 notches it cause stress on the fish and show signs of being sick. ( If your ph is right most likely your ammonia level should be straight, I would also check out that ph contol device)
