Is this a cycled and bedded in tank ? Do you have any water test results for us. Could be ammonia burn but I often see darker colours when ammonia is the issue.

Suggest checking the water quick. Looks almost like beginnings of velvet.

All I could suggest with the lack of info, maybe adding some aloe vera based comfort meds into the water, the only goal with this is just to relieve some pain really, I'm leaning towards ECW's post as above:/

Looks either fungal or an extreme reaction to something in the water like slothing of slime coat.

I would do a full test of all water parameters.
If something doesn’t check out right with the water I would recommend water changes to quickly help to correct them.
I would add tea spoon of salt per gallon slowly.
Turn the lights off to reduce stress.
Has anything been added to the tank?
Any background information you can offer would help.

I just remembered. I’ve had stuff like that happen to these two fish that I can’t remember what kind they were right now. They would develop that same fungusy growth. At the first sign of that growth it meant a big water change was needed. Hours later the growth would be gone and the fish would be fine. Try a big water change. It may be the same thing that affected those fish when the tank was overdue for a water change. When’s the last time you changed his water?

I’ve seen this type of reaction to medication.
I also seen this happen to fish where PH test regent was accidental added to the aquarium.
I agree with the water chang suggestion.
I also see some fin damage that makes me think this could be the culmination of a longer term problem.

I agree. Look at the emaciation/sunken in belly and 45 degree body positioning. Poor guy.
