Help poly is sick

Hi guys my ornate has been sick for 2 months.
the ornate was like this after an ammonia spike. which has been corrected long ago. However he still wont eat. and cant move properly he can only move his head. I moved him into a small breeder tank awhile ago becos its quite hard to feed him and take care of him in my 125 gal. also now his spine looks as if it has slanted.
What can i do in order for him to become well again

Looks normal.... post a video. what have you tried feeding

Yeah he looks normal..But he cant move properly at all and he wont eat. Tried shrimp, beefheart, was going to try feeders but iam quite reluctant. He ate shrimp before. that was his daily meal

I'd try small live worms and have the lights off. It will be close to the ornates mouth. It's worth a shot to try to trigger an appetite
