Help possible hole in head!!!!

So I recently transferred my Oscar who out grew his 40 breeder into my 180 and he developed a weird white spot on his head I thought maybe it was the start of hole in head disease I've heard about this especially in Oscars but his is not quite that bad due to it possibly still developing or maybe he trashed around a bit and hit himself on a peice of Rock in the aquarium I'll attach a pic can anyone tell me what this spot is he's definitely my favorite fish and I'd love to save him if possible!! Should I just wait to see if anymore develop and then treat or should I treat Imitately?? I really just need to know if it's Hexamita or somthing else and just incase anyone's wondering he does have a crooked lip and he is solid White making it harder for me to tell what this is myself.

looks like a bump to me to, to be on the safe side keep nitrates under 20ppm if not already.

Just a tiny wound.
Hole in the head looks like actual holes, or circular wounds.
 
it will also have a white like light fungus growth around the edges.
