HELP SEBAE DYING!!

Hey guys I have this sebae anemone and just the past couple of days its sucked inside itself. I went to move it and it has a white tissue type stuff coming off of it so I took it out and put it in a clear bag inside the tank what should I do or its there anything ., now also three days ago I did a water change with r/o water so I don't think that should have affected it.
 
Without very detailed history of the tank and it's parameters any diagnosis would be wild speculation at best.

With no experience myself I can only tell you what I have read after extensive research in prep for the the nem tank I am currently building. I found the only way anyone has kept any anemone alive for more than several months (fewer than 5% of those who try) is by perfectly regulating and providing ideal conditions for them, because if you slip up once your nem could be toast in the blink of an eye. Most successful long term anemone keepers usually build their tanks tailor made for the keeping of whatever animal they are trying to grow, in addition to providing diligent and proper husbandry.
 
Sounds like its to late, once nems get to this stage they are usualy well beyond recovery. How is your lighting? Did you feed him regularily?
