So my bass has been rolling around and can't seem to stay balanced? He is 1 year old and in perfect conditions. Ph 7.0 nitrate 0 nitrite 0 ammonia 0. Temp 81-83 heavily planted aquarium he only eats frozen fish. Weekly 25% water changes R/O water only.

The RO water could be the problem, it could be taking all the bacteria out of the water.

The aquarium also has a very large sump and has been running for 4 months now. The media was already cycled from a different aquarium. Does any one know of some thing that would cause a fish to tilt. He no longer stays righted. He rolls and is always tilted. At first I thought it could be something with his swim bladder but he suspends just fine. But he won't stay straight. He is always tilted like he is leaning to his right side.

With the information you gave us the only thing that comes to mind is a parasite, but even then most frozen food are blasted with strong UV lights to kill all parasitic/harmful things. You could narrow it down to an internal parasite but like I said it's doubtful, to be honest I think i'm missing some information here.

RO/DI water is a premium for Cichla.

X2, but I highly disagree with RO/DI being premium for cichla.
 
Where I live the tap kH/gH is as hard as rock, and in my agenda of Cichla keeping I try to get as close to their home conditions as possible. And to my understanding most of the SA river systems Cichla are found in have softer water profiles. Premium was probably the wrong word to use.
 
I agree with parasites just had this happen to me a few months back and they didn't make it. If you caught it early enough you might be able to save him good luck.
