I can't tell you for sure. It may be an infection of some sort, but it could also be constipation or maybe eggs. I haven't been around spawning Oscars so I don't know what an eggy female looks like.
Need more info. How much do you feed him, what foods, and how often? How low do you keep the nitrAtes? Temp of tank? Tankmates? Any unquarantined or live fish fed lately?

If your Oscar is eating OK, maybe it is constipated. You could stop feeding it for about 2-3 days; and them offer to you Oscar some boiled peas (without skin). You can also give to it a couple of olive oil drops.
did you feed him recent to this picture? That could be nothing more than a large gas bubble in the stomach, constipation, ect. Look for the next rest stop and see if anything unusual is in the fecal matter.
