Help with diagnosing potential disease

Hey guys I'm new too this forum and i was wondering if you could help me in figuring out what the issue is and what steps I need too take I have 3 geophagus Surinamensis but only one has what I can describe as what look like black seeds scattered round the inside of its body none of other geos have it and I have trawled the internet with no results it also has a lump on the side of its water parameters are 0ppm of ammonia and 0ppm of nitrite and 20ppm of nitrate IMG-20160715-WA0001.jpg
 
Those black dots in the picture just seem to be a stressed out coloration, the lump might have been from a nip by another fish but I can't tell really from the picture. Just keep up water quality all parameters maintain as 0, what are the tank mates?
 
His diet is varied he is fed on blood worm shrimp cichlid gold too name a few when he hasn't got those stress lines he still has those black dots on his body
 
Sorry I forgot too mention his tank mates he's in there with two other Surinamensis and a large white stripe geo and a royal pleco but the large geo is not interested in them
